As a patient at Summerhill Surgery, you have a legal right to ask for a copy of the personal information we hold on you. Asking for this information is known as making a Subject Access Request (SAR). You do not need to explain why you want the information, and in most cases the information is free of charge, however, this is only for the first copy. After this, there may be a charge.
Summerhill Surgery will respond within one calendar month of you making the request, however, if the request is complex then this time could be extended up to three months. We will inform you if this is the case within the first calendar month.
We may request that you provide proof of your identity, for example, a driving licence, passport, a marriage or a birth certificate. This is to ensure that we are providing the correct person with the correct records, further protecting your data.
You are able to request a specific piece of information, or all of the information that we hold about you.

Please include the following:
- Full name
- Address
- Date of birth
- NHS Number
- Details of your request.
