You do not require a
Doctor’s sickness certificate for
any illness lasting seven days or
less. Your employer may, however,
require you to complete a
self-certification form (SC2) that
is available from your employer.
For any illness
lasting longer than seven days you
will need to see the Doctor for him
to issue a sickness certificate
(FPmed3). Any extension can be
requested in writing, you do not
need to see a Doctor.
Please note
that should an FPmed3 be required
for the first seven days of any
illness, a charge of £20 will be
made.
