How Do I Replace my Medical Card?
With effect from
March 2007, the Kent Primary Care
Agency will no longer be accepting
telephone requests for replacement
medical cards.
If you have lost you card, or wish
to have a replacement showing new
details, you must make formal
application in writing to them.
