If you are new to the
surgery you will be invited to
attend for a registration medical,
which includes filling in a form
about your past and current medical
history, a short medical
examination, including height/weight
and blood pressure. Please bring a
fresh specimen of your urine and
your medical card (if you have one)
with you.
This service is also extended to
patients not seen in the surgery for
3 years.
You have the right to express a
preference to receive services from
a particular clinician and we will
record this preference. We will
endeavour to comply with this but
please note this can not be
guaranteed.
